Research progress of continuous nursing care in patients with percutaneous coronary intervention

To review the application of continuous nursing in patients with coronary intervention, studies on continuous care for patients with coronary intervention were reviewed. It can be found that continuing care can provide timely and effective advice for patients, improve patient compliance, and self-management ability and patient satisfaction. Simultaneously, it can reduce the risk of rehospitalization, save manpower, material resources, and financial resources. Moreover, continuous care should develop relevant theoretical frameworks and standard operating procedures to make it more scientific in patients with coronary heart disease.

Advances in the Relationship between Apolipoprotein Gene Polymorphism and Blood Lipid and Cardiovascular Disease

Dyslipidemia is a leading cause of cardiovascular disease. At present, studies have shown that the incidence of cardiovascular disease in our country increased year by year. According to?WHO statistics?in 2013, it showed that about 17 million people worldwide die from coronary heart disease (CHD) every year[1]. Currently, CHD is the first cause of death in western countries and the incidence of CHD also showed a trend of increasing[2]. In recent years more experts and scholars at home and abroad found gene polymorphism is closely related to high-density lipoprotein cholesterol (HDLC) gene and triglyceride (TG) levels. Apolipoprotein (APO) gene is a kind of popular polymorphic proteins, whose genetic polymorphisms is through the impact of lipid metabolism, and then closely related to cerebrovascular diseases. But the results are different in different populations and races, or even the opposite. This review will summarize the gene polymorphism loci of common apolipoprotein-ApoA1, ApoA5, Apo B, Apo C3, ApoE, which is associated with lipid levels and cardiovascular disease. It is important for us to get a further understand and prevent the occurrence and development of cardiovascular disease from gene level.
Efficacy and safety of Mahuang Fuzi Xixin Decoction on sick sinus syndrome: A meta - analysis

Objective: To evaluate the efficacy and safety of Mahuang Fuzi Xixin Decoction on sick sinus syndrome and provide evidence for clinical practice. Methods: Randomized controlled trials of all the languages of Mahuang Fuzi Xixin Decoction on sick sinus syndrome were collected by computer search and manual retrieval. The retrieval time was from January2000 to January 2017. According to the inclusion and exclusion criteria, 2 reviewers independently selected and extracted data, then evaluated the quality, cross-checked the information and evaluated the quality of menthodology. Through discussion or third reviewer to help solve the divergence, RevMan 5.3 software was used to perform meta analysis. Results: A total of 7 documents (n=612) were finally enrolled, with358 in Mahuang Fuzi Xixin Decoction group (treatment group) and 254 in control group. Meta analysis showed that the treatment(86.9%) was more effective than the control (70.1%), the difference was statistically significant (RR = 1.25, 95% CI:(1.15-1.37), P<0.001); the treatment (17.0%) was safer than the control (49.8%), the difference was statistically significant (RR=0.23,95% CI:(0.06-0.93),P=0.04).Conclusion: The existing clinical studies suggest that Mahuang Fuzi Xixin Decoction on sick sinus syndrome is effective and safe; due to the limited quality of the enrolled documents, the above conclusions need more high-quality randomized controlled trials to be verified.

Analysis of 610 cases inpatients with chronic heart failure

Objective: To analyze the clinical characteristics and medical treatment of hospitalized patients with chronic heart failure, and provide medication for the patients. Methods: According to medical records of inpatients, demographic information, etiology, clinical features and treatment information were collected. Results: A total of 610 cases with heart failure from our hospital between July 2010 and June 2016 were analyzed. The average age of all the patients was 63. Males accounted for 50.49%.There were 82.7% patients with NYHA functional classification at III-IV. 31.3% patients with coronary artery disease, 28.4% with rheumatic heart diseases and 21.8% with expansion of heart disease. 27.4 percent of the patients with the left ventricular ejection fraction lower than 40%.There were 78.9% patients received aldosterone antagonist treatment, 63.4% received ACEI / ARB, 62.1% received digoxin, 59.8% received beta blockers, and 75.4% patients received decoction, 80.8% received Chinese patent medicine. Conclusion: The study have shown that high utilization of Traditional Chinese medicine is to highlight the advantages of the combination of Traditional Chinese and Western medicine in the treatment of heart failure. The usage of spironolactone, ACEI / ARB, beta blockers in our hospital was lower than the guideline ratio, higher than that of the national average. However, the use of diureticis is lower than that of national average, which may relate to the use of damp-clearing herbs. Clinicians should pay attention to the high utilization rate of digoxin. Patients with the left ventricular ejection fraction > 50% accounted for 61.1%, which required more attention should be paied to the diagnosis and treatment of patients with heart failure of the ejection fraction remains.

PtDA can improve the patient's understanding of cardiovascular disease and the potential risk of perception of cardiovascular disease, as well as improve the communication between doctors and patients. The continuous management in symptoms, risk factors and lifestyle behaviors is needed for a good prognosis for cardiovascular disease.

Abstract

This article introduced the development and application of patient decision aid (PtDA) for cardiovascular disease. There are many risk factors for cardiovascular disease, and the mortality rate is high. Therefore, continuous management in symptoms, risk factors and lifestyle behaviors is needed for a good prognosis. PtDA can improve the patient's understanding of cardiovascular disease and the potential risk of perception of cardiovascular disease, as well as improve the communication between doctors and patients. The emergence of narrative evidence-based medicine provided a new opportunity for the further development of PtDA. At present, there were demands for domestic cardiovascular decision aids, but the researches were extremely limited.

Effects of hypoxia on sodium current in rat cardiomyocytes

Abstract

The aim of this study was to investigate the effect of hypoxia on the sodium current of rat cardiomyocytes in order to explore ion channel mechanism of cardiomyocyte hypoxia. The rat cardiomyocytes were isolated by acute enzymatic hydrolysis. A group of untreated cells were used to record sodium currents using whole-cell patch-clamp technique, another group was subjected to hypoxia and record sodium currents using same technique. The results showed that the morphological trajectory of sodium hypoxia was not changed compared with that of normal cells. The I-V curve of hypoxic cells was significantly higher than that of normal cells, and the peak current of INa was 15.68% higher than that of normal cells (P < 0.0001). Activation potential of normal and hypoxia cells was about -40mV, the maximum peak current corresponds to the stimulation voltage of -25mV. The above results suggest that rat cardiomyocytes sodium current increases in the case of hypoxia.
